MILD Procedure
More room for the nerves, without implants or open surgery.

At a glance

The MILD procedure (minimally invasive lumbar decompression) treats lumbar spinal stenosis caused by a thickened ligament in the back of the spinal canal. Through a portal about the size of a pencil tip, small pieces of the ligament are removed under X-ray guidance to give the nerves more room. It leaves no implant and is done as an outpatient procedure. Dr. Benjamin (Ben) Klyachman, D.O., board-certified in pain medicine and physical medicine and rehabilitation, leads care at Institute of Function in Nassau County, NY, which welcomes patients from across Long Island, New York City and beyond.

Who it is for

People with lumbar spinal stenosis who get leg pain, heaviness or numbness when standing or walking that eases with sitting or leaning forward (neurogenic claudication), when imaging shows a thickened ligamentum flavum.

How it works

Through a small portal and with X-ray guidance, specialized tools remove small portions of the thickened ligament. No implants or stitches are typically needed, and patients usually go home the same day.

Evidence

In the MOTION randomized trial, patients treated with MILD plus conventional care improved more in walking and pain than those with conventional care alone. Results vary, and MILD does not treat every cause of stenosis.

Risks

Risks are uncommon but include bleeding, infection, a dural tear and nerve irritation.
